Please see the table below for a list of U.S. government and ECFMG fees that may be required for participants in ECFMG’s J-1 program. Note that not all fees listed are applicable in all cases or for all applicants. The SEVIS fee, in particular, is required only of initial applicants for sponsorship. Fees are subject to change.

EVSP will send Form DS-2019 through the United States Postal Service (USPS) using standard first-class mail service (i.e., without tracking). If faster and trackable delivery is preferred, a pre-paid, pre-addressed shipping label may be generated online through an express shipping courier and uploaded at the time of application or duplicate request. Shipping labels from USPS, DHL, FedEx, or UPS will be accepted; however, please note the following:
All labels must be formatted as follows:
"TO" FIELD: For shipping labels uploaded as part of an application for sponsorship (initial or continued), the label must be addressed to the TPL at the host institution. Shipping labels uploaded outside of the application process for a duplicate Form DS-2019 can be addressed to either the TPL or the J-1 physician for whom the request is being made.
"FROM" FIELD: All shipping labels uploaded as part of an application or a request for duplicate Form DS-2019 must be populated with Intealth's address in the "From" field. Intealth's address is 3624 Market Street, Philadelphia, PA 19104.
EVSP will send Form DS-2019 by express courier service ONLY if a pre-paid, pre-addressed shipping label is uploaded to the applicant’s on-line record through the On-line Applicant Status and Information System (OASIS) at the same time as all other required supporting documentation. If not provided prior to application review, use of an expedited shipping label cannot be guaranteed.
